How do I request a new form or changes to be made to an existing form, in DocuSign?

If you notice there is a form missing or you need a change to be made to a form, you will need to reach out to your MC Leadership, and they can fill out and submit the form in this link: tiny.cc/kwforms.

Every effort will be made to ensure that the requested changes are fulfilled, however if there is a request that changes the overall structure of a contract then unfortunately that change might not be possible.
